For non-emergency care, contact Teladoc first.  All optional inpatient and outpatient surgical and diagnostic treatments need pre-authorization. Nova will cover any reasonable and customary amounts. To avoid balance billing, get a Pre-authorization first, particularly if the amount exceeds $300.
